the article states that he was carrying a holstered gun... to me, this means it was in his hand(s)



this kind of thing makes me question carrying condition 1.

if i were to have an accident like this and my wife or daughter were caught in the path... i'd lose it.

With some guns, perhaps. But with a 1911's grip safety and thumb safety it's not really an issue. Even if you did grab the gun on the way down, you would have to somehow manage to disengage the thumb safety, depress the grip safety, AND pull the trigger. I wouldn't even want to try to calculate the odds on that.

So we know it was not a 1911 carried in correct condition 1 (at least).
