Someone double check me please...

I'm getting ready to send all my forms tomorrow.i have:
1.certificate of training form
2.2 affidavit forms
3.photo holder/id signature form
4.barcode copy with checklist
5.2 photographs
6.fingerprints were done today through "fast"
Am I missing anything??

CHECK LIST:

1. Verify no missing information on:
a) fingerprint cards
b) affidavits
c) passport photo holder (signed in black felt tip pen)
d) application

2. Be sure photos:
a) are signed by fingerprinter and applicant
b) are taken with a light background, preferably white or off-white background

3. Be sure to include all required documents in packet (See DPS checklist in packet):
-2 fingerprint cards signed in proper boxes by applicant and person taking prints (one for renewal)
-3 notarized affidavits
-1 passport photo holder w/ 2 signed passport photos
-Completed application form
-CHL 100 (be sure you sign the CHL-100, not just the instructor!)
-Personal or Cashiers Check/MO in appropriate amount, unless paid on Internet

TIPS (Not technically required, but good procedures.)

1. Put your application number on each page you turn in. (Many new applications don't have a number, unless you applied online.)
2. Send application by method that allows you to prove when it was received. (Ex. FedEx, certified mail, return receipt requested, etc.)
3. If you were born outside the United States and are an American citizen, send a copy of your birth certificate and passport, if you have one.
4. Military applicants:
a) If applying for reduced fee because of past service in the military, include a copy of your DD214
b) If active military:\

1) send a copy of your military ID and your orders or LES
2) send a copy of Forum 522 to prove you qualified within five years

Suggestions from members' experiences:

1. Re: DD214s: Be sure to use an 11x17 copy or copy from the top and bottom of your DD214. Mine did not include the line that listed the Honorable Discharge and they requested a new copy.

Since you indicate that you're sending the "barcode copy with checklist", you have done the on line application. Having applied on line , it is not necessary to send in the "2 affidavit forms", as they are part of the on line application. The CHL-85 and CHL-86 Affidavit Forms are only required to be mailed in with paper applications.

If you have a Texas Drivers License, it is not necessary to send the "2 photographs". Write the words "Please use Texas Drivers License Photo" on the CHL-6 Photo Holder, and DPS will use your Drivers License Photo.

Yes, I know that the checklist calls for affidavits and passport photos, but this is just another example of where DPS has not changed the forms to match their current practice.

Use the checklist to ensure that you are submitting anything else that is required, e.g., DD-214 for anyone claiming the veterans exemption.

Finally, I recommend that you mail the documents via the USPS rather than uploading through the DPS web site. It may be my error, but uploading has not worked for me on the three times I tried.
