Just list the best you can. You can go to each jurisdiction and get traffic convictions, but I wouldn't waste the time.

Minor in Consumption is a Class C Misdemeanor, and does not count Your Class B Misdemeanor conviction date needs to be more than 5 years ago. Probation dates don't matter.
