Apollo 13 service module
...
I didn't realize it at the time, being only 10 years old, but the Apollo 13 mission was probably one of the worst periods of my dad's life. He was a government quality assurance inspector at Allison in Indianapolis, IN, where the titanium fuel tanks for both the service module and lunar module were made, and he was part of the government team that inspected them. When there was an explosion in the SM, an obvious possible source was the fuel, and everyone was wondering if they had done something wrong or missed something. My sister later told me that Dad threw up in a wastebasket when he first got the news (we got a phone call at home about it). This really shocked me, as Dad was a veteran of the Depression and WWII, and was not at all prone to displays of emotion. It was quite awhile before the oxygen system was determined to be the source of the problem, much to his relief I'm sure.
