Investigators issued an unusually detailed report of an incident that occurred in the Greeley, Colorado, area in November.

A man took the keys of his girlfriend's car by force and drove off. She reported the incident to the police as a car theft.

Meanwhile the man called his mother and went to her house. He fled when police attempted to apprehend him.

His mother reported to the police that he was possibly suicidal and "didn't want to go back to prison."

Somewhere along the line, a dispatcher erroneously escalated the incident to "armed and dangerous."

After a protracted chase, police stopped the vehicle. The suspect lunged at the police, five of whom engaged in a close-contact melée. The suspect gained control of an officer's service piece, a .45-caliber Kimber, and fatally shot the officer.

Another officer fatally shot the suspect.

The shot that killed the deputy first struck his ballistic vest, was deflected upward, and entered his face.

A total of six shots were fired, all at close-contact distances.
