I'm going to go out on a limb here and say 1. Gun not pointed in a safe direction, 2. Finger on the bang switch!

It would be good to have more details about how it happened. Rule number one is to keep the barrel always pointing down range. Rule number 1 B is never sweep a person with the barrel.
