He was disqualified for the flag patch exceeding the size set by the rules, not for wearing an American flag patch.
There are consequences to breaking the rules. Shame on both coaches, the swimmer’s for not ensuring his team was complying with all of the rules, and the opposing coach for being such a little man as to get his team a win in this manner.

I see the point about rules, but why couldn't they just require that he not wear the cap? Want to take up bets that they were Democrats?

I agree with Grayling813 that it was not the American flag but the size of the patch that caused the problem. I also agree that a good amount of the blame falls on the teen's coach for not checking his team's equipment.

But the opposing coach is even worse than it appears at first glance. Not only was this a poor way to get his team a win, but he did it only after he knew his team lost. He did not complain about the patch or ask for it to be measured for compliance until after the race. This is not the sportsmanlike conduct that it is his job to teach the competitors.
