I'm new here, but I'd be willing to bet this has been brought up before (search engine?).

As far as example 2, that indeed used to be the case. You bring in a six shooter/qualify, then your licence will not have "SA" designation on the back. I believe this might have changed, I'm sure someone more knowledgeable will chime in shortly.

As far as where it comes from, well, more appropriate question is where does it not come from.

Thankfully, we have multitude of chl'ers who have over the years worked and continue to work steadfastly to set the record straight.

Forgive me if I'm wrong, but if I recall correctly, you used to have to use a .32 caliber or larger to qualify, although you could always actually carry a .22. Again, this is likely something where there was a smidgen of truth and folklore took over.

.32 is the minimum caliber I believe.

Minim caliber for qualifying is .32 for semi automatic, and .38 for a revolver. Also, there is no restriction on the caliber you carry. These answers are per FAQ #45 on this link.
