Employment Address for Working from Home

Employment Address for Working from Home

Basically what the title says. I had a short employment working from home for a company doing reservations and such. I only worked there a month before I had to "resign" because my internet at the time was horrible and caused frequent disconnections. Anyway I'm curious as to what address I should list on the application. Should I put (worked from home) next to the company name and my home address or the company's HQ address which is located in Arizona?

I would use the employer's HR address. When it comes down to it, that's where DPS would want to contact them anyway, I would think.

I work from home 99% of the time, and I always use my company's HR address for such things -- it's always worked out well for me.
