Not with this one, but I haven't had it that long. Couple months. I've owned one off and on since I was 18 and I can only ever remember one stove pipe style ejection issue. I've never had one completely fail to extract, and only the once case get stuck sticking out the side. No telling how many thousands upon thousands of rounds through them. Think I was like 22 or 23 when it happened. Had a few thousand rounds through it and never been cleaned or lubed. So not exactly a surprise it happened.
