A little over a year ago I applied for a CHL and on the application pointed out that I was treated for depression 24 years ago (being the honest person I am). After several weeks I received a request asking for clearance for me from the doctor who treated me. I was going through some health issues when I received the request and just let it go.
I'm better now and again I'm thinking of applying again.
The question: Can I just get clearance from a different doctor?
The details: In 1992 I was the managing broker of a Century 21 office in Colorado and I had 2 heart attacks within 6 weeks. I was removed as manager (for my own good) and had no insurance. Within the next year I had declared bankruptcy, been kicked out by my girlfriend and was broke. Not surprisingly I was feeling pretty down. I finally went to see a resident doctor at the University of Colorado (Colorado indigent program) and took anti depressants. After a year (approx.) we both agreed I no longed needed the medication.
Currently I've been at the same job (which I love) for 15 years, a wife that I adore (married 10 years), in a state that I love (Texas), I've become a born again Christian, bought a wonderful house 9 years ago and I'm pretty much one of the happiest guys you'll run into. But the doctor I saw was a resident and moved on many years ago. I don't even remember his name. I don't think there's any way I could locate him and even if I could, he wouldn't sign anything without seeing me.
Is there any way to appeal to a board or something to get it waived? Or can I go to another doctor and get a release?

I'm not aware of any requirement that it be from the same Dr as the original diagnosis. I'd recommend getting an appointment with a mental health provider local to you, explain your history & current situation, & have them write the letter / fill out the form.
Does the form instructions you received from the DPS or Med Advisory Board say it must be filled out by the same Dr as the original diagnosis?

I fear that these restrictions (and the potential for further restrictions in the future) cause a lot of folks to not seek treatment for mental issues they may have.
