Tried to look for info on this forum and on the web. What I came up with was a "Passport Photo Holder" is actually a form available at http://www.txdps.state.tx.us/InternetFo ... /CHL-6.pdf" onclick="window.open(this.href);return false; . Not 100% if something else is needed. I just wanted to verify with you good folks.

In my instructors manual from the DPS, they also state the photos and photo holder be enclosed in an envelope with your name and "Photo's Enclosed" written on the outside of the envelope. Since they do not require signatures on the photo's, I presume this is to allow them to keep track of names/photos.
