Chl-6 Form - How many photos?

I just completed my CHL class, and the instructor said us out of state people had to submit two copies of our photo with the form. The form only says one copy, however. Which is it? And does it matter if we cut it out, or leave it on the paper from the photo place?

Welcome to the Forum. Send two passport size photos taken against a white background. Don't attach them to the "photo holder;" put them in a small envelope
