This holster is called the Don Hume PCCH model which stands for Preferred Concealment Carry Holster.
This fine holster is designed to be carried inside the waistband strong side behind the hip. It is made for the right side - it has a wide belt loop with two pull-the-dot black snaps that function independently. It will fit belts up to 1 3/4". Reinforced top opening for easy re-holstering, and form molded fit. A body shield provides comfort between the weapon and user.
This is an excellent concealed carrying rig and has never been worn. I also have a Don Hume D417 Single Magazine Carrier (Saddle Brown).
