CHL Checklist Check Before Submitting

First time submitting for my CHL here is what I have:

Confirmation Email with Barcode, Finger Prints Receipt, Chl-100 Training Form, A Color Copy of my Military ID Front and Back, And my Leave and Earnings Statement. I have a Texas Drivers License but, do I need to send a copy to them as well?

I would like to know if that is all the info I need and also a 1 side question. Is my CHL application ID# the # on the bar code in the Confirmation Email?
